Frequent Urinary Tract Infections

                Recently I’ve been seeing a surprising number of women with chronic or recurrent urinary tract infections and I wanted to share some observations that might help prevent other women from having to deal with similar issues.  For many of these women, the symptoms resolve only when dealt with on a physical, mental, and emotional level.  They are a great lesson to us as patients and as doctors in truly addressing the whole person.  Here is a sample of observations on all three levels that may help with prevention.

Physical:

·         Sweets and simple carbohydrates (including just too much fruit or juice) raise the blood sugar and suppress the immune system leaving you susceptible to infection.

·         Repeated antibiotics (for past urinary tract infections or other issues)

o   destroy the good bacteria in the digestive tract, which interferes with immune function

o   leads to leaky barriers in the urinary tract as well as the digestive tract and can cause increased inflammation of tissue, making it more susceptible to infection

·         Repeated infections generally lead to angry tissue throughout the urinary tract.  This tissue is easily irritated and patients may experience symptoms even without an actual infection.  Breaking the antibiotic cycle is very important in getting rid of chronic infections.

·         When we don’t drink much water, we don’t flush bacteria out of the urinary tract as well or as often.  Staying well hydrated is the best prevention.

·         Many of these infections occur after sexual activity.  Urination within a few minutes following sexual activity is an important prevention for urinary tract infections.

Mental:  Too much intellectual activity or analytical thought can disconnect us from our bodies and cause an imbalance.  Exercise, especially involving the pelvis, is a great way to shift out of the head and into the body.  Consider dancing, hula-hoop, or yoga, but any physical activity is good.

Emotional: Please note that I am not implying that urinary tract infections are purely emotional issues, rather that there is an emotional component to most physical illness and these emotional patterns come up often in people who are prone to urinary tract infections.

·         Every organ and area of our body is associated with a primary emotion.  The pelvis in general is about safety.  Any issues about safety, particularly as they relate to intimacy, will often manifest in the form of discomfort or disease in the pelvis.  All patients with frequent urinary tract infections should carefully consider where they might feel unsafe.

·         Bladder emotions: the bladder is often associated with fear and shame.  These emotions are often important to work through for people struggling with bladder related issues.

When I look at my patients from all these perspectives, I find that there are many tools for addressing the underlying cause of chronic bladder and kidney infections.  Through dietary changes, herbs, counseling, homeopathic support, and hands on medicine, we are able to support the whole person, creating the environment for a healthy bladder.

Dr. Molly's Word

1.  Angelina Jolie's decision:  It is all the talk around the world about Angelina's decision about preventative masectotomy. I applaud her decision. With her family genetics, having this type of surgical intervention and having the ability to do reconstruction is certainly less toxic then the anxiety of waiting for the other shoe to drop and then poisoining yourself with chemo and radiation.  If I had had the choice this is what I would have chosen.  It is just sad that the rate of breast cancer is so prevalent. It makes me contemplate the poisoning of our environment and the effects even on our DNA.

 
2.  Vitamins and expriation dates:  I am being asked a great deal about our client's vitamins and expiration dates.  Here is a general guideline:  Expiration dates mean that the strength of that product is good until that date.  So expirations on supplements, unlike food, do not mean they are harmful to you.  It just means that it might be less effective.  Now having said that there are some caviots:

a.  minerals never out date; they are rocks.  Unless there are other things in the product that might deteriorate they are great.

b. homeopathic remedies and herbal tinctures never outdate.

c.  Things that can be hurtful:  oils can go rancid.  How do you tell?  They smell bad like bad food.   Vitamin C and B vitamins will oxidize and they will speckle and turn color.  Not good for you.

d.  Herbs get less potent but not harmful, generally.

So do not throw out your old supplements necessarily, just realize they may not be as effective so you need to take more to get the desired outcome.


3.  Car Accidents:  Please call Emerald City Clinic first (after 911 if necessary) if you have had a car accident.  We are the BEST for managing your PIP claims and look at the whole picture.  Also the sooner we get our anti inflammatories and nutritional support to you, the better the results.   We then manage the whole picture referrring you to the best physical medicine treatment options for your particular case.   We are your prirmary care physicians and we look at all aspects.  People often go to the chiropractor but they will not document insomnia, fatigue, change in menstruation, etc that are all possiblities from a car accident and should be documented thoroughly.  Please get in to us with in 48 hours and same day if possible.

Alternative Treatments in Cervical Dysplasia and HPV by Dr. Rachel Erickson


Many women are getting scared by their abnormal PAP results and their doctors’ nonchalant responses of, “let’s retest in 3-6 months”. Another group of you are panicking because your doctor immediately wants you to get a colposcopy and a LEEP to solve the problem. 
In the medical community at large, “cutting it out” is a common practice when it comes to abnormal tissue. In my practice we have a lot more options. (This is true in general of most conditions)
A standard routine of care for a healthy female with cervical changes is a 3 pronged approach:
1.       Dietary changes to help rid viruses
2.       Supplements designed to help your body heal
3.       Suppositories to treat right at the source
Oftentimes there is nothing more that is needed and by their 3 month retest their PAP is clean!

You are what you eat! by Dr. Vanda Huang

I found this photo gallery on what the world eats in a week so interesting:Hungry Planet: What the World Eats. I had no idea people ate narwhals, or polar bear for that matter! It was interesting to see the proportion of processed food to fresh produce from country to country as well.

And somewhat related to that is an article in the Atlantic about how "Serving Convenience Foods for Dinner Doesn't Save Time." It makes the excellent point that home cooked meals from scratch organically bring the family together for meals since with convenience foods you can easily eat whatever, whenever, which means you most likely won't be sitting down with the family when you eat. Eating in front of the television, in the car, or on the go, tends to encourage mindless eating. Eating in a slower, relaxed setting while socializing and interacting with your family or friends actually helps your body absorb more nutrients from your meal- we tend to chew more and take our time which leads to better digestion. And of course, beyond the nutritional benefits, bonding with your family during mealtime makes home-cooked (vs convenience) meals even more worthwhile.

The "Bear" of Overcoming Stress to Increase GI Health, by Dr. Erin Westaway

Healthy digestion is one of the foundations of our wellbeing and one of the areas of our health that is most directly affected by stress.  There’s a reason we talk about not being able to “stomach” bad news – in times of stress, we literally shut down the signals to the stomach to digest.   

The body diverts energy and blood to the areas that are needed to deal with an acutely dangerous situation, like running from a bear.  We need to be able to use our big muscles, see clearly, and think fast, so blood is sent away from the digestive tract to the large muscles, the heart, the lungs, and the brain – not to the stomach and intestines.  At the same time, the nervous system sends signals to the digestive track telling it not to waste energy.   

This is all great if you’re actually running from a bear.  If you run from a bear you either get away from the bear or you get eaten.  In either case, the stress is over fairly quickly.  For many of my clients, however, the stress of life causes them to respond physiologically as though they are always running from the bear.  This chronic stress causes them to rarely use their digestive tract efficiently.  Decreased blood flow and neurological signals to the digestive organs leads to poor digestion and a buildup of unhealthy amounts of half-digested foods.  This causes fermentation, gas and bloating.  It can also lead to constipation or irregular bowel movements, some of which may be loose and urgent. 

So, what do we do about it?  It’s unlikely that you’re going to remove all the stress from your life.  What you can do is change how you respond to stress and help your body shift out of that “fight or flight” mode.  Try some simple tips at your meals for the next week and see if your digestion changes.
 

1.       Avoid stress during mealtimes as much as possible.  This means: shut off the television, do not eat at your desk, save stressful topics of conversation for another time.
2.       Pause before you eat and try this exercise for 5-10 minutes:
a.       Close your eyes and feel your feet on the floor.
b.      Slow down your breathing, particularly your exhale
c.       Don’t take deep breaths, but concentrate on exhaling slowly.  If this is difficult, purse your lips slightly and pretend you are exhaling through a straw.
d.      Allow your breath to shift from your chest into your abdomen
3.       Slow down!  Aim to chew each bite at least 20 times.  You don’t have to do it perfectly, but the idea is to slow down and experience the food in your mouth.  Tastes, smells, textures, and chemical and mechanical signals involved in chewing all help to shift your body into a state that stimulates digestion. 

For many of my clients stress is a very real cause of many of their digestive symptoms.  This does not mean that their symptoms are “all in the head” or made up.  It means that the way they experience challenges in their lives is causing significant changes in the way their bodies function.  The above simple steps, along with those that help to reduce stress, will start to shift the body back to a more functional state.  At times other therapies like biofeedback, bodywork, homeopathy, and herbs are also useful in addressing the effect of stress on the body and require the assistance of a qualified health care practitioner.  You can get started with the tips above on your own today.

Pillars of Digestive Health

If you have ever seen a naturopath, you have probably realized that much of your health has to do with how well you digest your food.  There are many components to this, but the basic pillars of digestive health are balanced gut flora and the production and excretion of your stomach acid and digestive enzymes.

 In order to keep your flora in check you can eat fermented foods like pickled vegetables, yogurt, kimchee, kombucha, or kefir.  If you don’t have a palate for the sour side of things, then rotating through different probiotic supplements is also a good way to support your digestive health.

There are also several things you can do to get your digestive juices flowing.  Squeezing a lemon into your water and drinking this before a meal will stimulate stomach acid secretion.  The same is accomplished by drinking apple cider vinegar diluted with water before meals or adding vinegar to your salad.  The stomach acid increases the secretion of all of the other digestive enzymes so that your food will get broken down properly.  If this is not enough, your doctor can help you choose a digestive enzyme that is right for you.   Also remember that if you chew your food thoroughly, your stomach and intestines don’t have to work quite as hard to get your nutrients into your body.

These simple steps will lead to a healthier digestive system and thus improved overall health.

Emerald City Clinic has removed Celestial Teas from our shelves, and we encourage you to do the same. Recent tests conducted by EuroFins (www.eurofins.com/en.aspx), an independent analysis company, have found that 91% of the Celestial Seasonings teas contained pesticide levels higher than the US Federal Limits.

These tests were initiated by Glaucus Research Group out of California, who do have a great influence on short-sale stocks. They do provide a full disclaimer that they will, in fact, benefit greatly if Hain Celestial (the mother company) stock plummets (which it has after this research). In addition to Celestial Teas, Glaucus Research exposed numerous other products of Hain Celestial as being falsely marketed. What tea do we suggest? It seems that similar tests were run on Twinings Teas, and they were given an A in sustainability.

2013 FLU TIPS from Dr. Molly Niedermeyer!

My brother emailed me asking if I had gotten my flu shot and I quickly responded that I would not get one and do not advise it. Cancer clients are suppose to strengthen their immune systems because our white blood cells are often compromised and we cannot "afford" to get the flu. This is an on-going discussion with my oncologist and I tell him getting the flu vaccine is "not negotiable with me." I was listening to an interview with the CDC on the radio yesterday, and they say this is the "worst flu season in a decade." When asked,  "Why?" the CDC representatvie's response was:

1. the flu virus seems to mutate and get stronger

AND

2. we really do not know.

I DO!!!!! First, we keep creating super bugs with vaccines. Those bugs want to survive and mutate getting smarter and smarter about tricking our immune systems. Secondly, when you get the flu vaccine you commit a portion of your immune system to only recognize the "blue hatted flu bug" which the CDC guessed would be the flu bug arriving this winter last spring. So, you now have less flexibility in your immune system since 80% of your fighting cells are sleeping on the job when the flu virus arrives with a "red hat." I would rather have 100% of your immune system capable of responding to whatever comes. Thirdly, our immune systems are weaker and weaker from toxic exposure, poor diets, lack of sleep, stress, etc. Better prevention would be Molly's 5 Point health plan.... sleep, eat well, drink water, exercise (increases white blood cells) and commit to a basic supplement regime.

Having said all that, I want you to know this has been one of the worst flus I have seen. What we have found to work right now:

1. Stay hydrated!

2. Warming sock treatment (you can find this on our website: http://www.emeraldcityclinic.com/educational/pdfs/sock.pdf)

3. Homeopathic Bryonia or Eupatorium

4. High dose chinese mushrooms like astragelus.

5. High dose lysine (2000 mg/day).

If you are sick please contact your Emerald City Clinic Naturopathic physician immediately as it really makes a difference to have an ally through this process. This flu seems to have 3-4 days of high fever that weakens your immune system and turns into whatever illness has historically been your weakness: bronchitis, sinus infection, strep throat. Do not be mistaken it is a virus and will not respond to antibiotics.

Insurance Open Enrollment: How to shop for insurance

 

Insurance 101: Educate yourself!

Many of you will be shopping around for new insurance in the coming months, and I wanted to offer a few tips. When speaking with the companies, be sure to ask the right questions, especially if you are looking for good reimbursement coverage of your ECN appointments. Dr. Molly Niedermeyer, Dr. Kelley Barnes-Valdes and Dr. Erin Westaway are always considered out-of-network, as they are not contracted with any insurance companies. Please be sure to ask 1) What is my out-of network deductible (this is completely separate from your in-network deductible)? 2) At what percent is out of network reimbursed?, 3) What are my co-pay and co-insurance? The answers to these questions will greatly vary not only between different companies, but different plans as well, so be sure to do your homework!
 
If you’d like someone to do this homework FOR you, we recommend Sarah Freeman at Mutual Benefits: 206-709-2619. During a free meeting, Sarah will speak with you about what kind of coverage will work for you and your family, and then present you with multiple plan options that will suit your needs. If you work with Sarah, be sure that you still know the answers to the questions provided once you are established with a company so there are no future surprises.
 
Once you have established a company that you are comfortable with, be sure to pay attention to the explanation of benefits that you receive in the mail or online. Often times, the insurance company will deny claims for reasons that can be remedied, and you will receive reimbursement once corrected. Look for words like documentation, bundling, and yearly service already covered. Let your ECN doctor know, and we can correct the superbill that was given to you for reimbursement.

Immunizations: Adult and Pediatric

Recently a client of mine asked to have me address immunizations in my blog.  I am happy to address any health issues that you would like me to.  Just email the office at frontdesk@emeraldcityclinic.com and we will our best to address your concerns publicly.  So here is a controversial topic of immunizations.  Public Health would not like my opinion about immunizations and therefore most Naturopathic physicians shy away from a stance because it goes beyond health into the political arena.  Given this environment I have never been one to shy away from an opinion.

First immunizations MUST be an individual decision between a client and the best medical advice that your physician can give you individually NOT societally.   It is a societal decision to immunize everyone and now it is societal health problem that everyone is allergic to eggs.  So I will attempt to discuss some of the "Fall immunizations".  First all flu vaccines are decide in the Spring as to what MIGHT show up and there is a great deal of money involved.   Flu vaccines generally do NOT improve your immune system and has some potentially life threatening side effects.  There are more health provoking ways to prevent the flu.   Washing your hands,  eating a good diet, sleeping, taking care of your immune system.  Once you have been exposed we have powerful herbal and nutritional medicines to prevent the horrible systems of the flu.  So generally speaking I do NOT recommend these annual flu vaccines.

Pertussis, otherwise known as whooping cough, is now being recommended for all adults to prevent spreading it to pediatric population.   The fear based advertising out there really irritates me.   If you are around infants under 2 routinely I might seriously consider getting immunized but it is NOT a given.  Pertussis vaccine does NOT give life time immunity and the disease does.   This is a "hot button topic" worthy of a John Stewart episode but I am not going to go there right now, I would just recommend that you talk to one of your Emerald City Clinic physician before accepting any immunizaitons.

Pneumonia immunizaiton:  Do not do it.  No reason.  Build your immune system.   Sleep.

Shingles:  Boy this is really an exercise in restraint for me.  A good case of chicken pox as a child will give you life time immunity.  Chicken pox vaccine given to pediatric clients instead of getting a case of chicken pox is resulting in higher levels of shingles.   If you never got a "good case of chicken pox"  and you are exposed to chicken pox as an adult AND you are stressed out and nutritionally deficient you can get shingles.  But shingles is easily treatable with Naturopathic care and a sign that your immune system needs some attention.

The bottomline is NONE of these immunization are getting to the cause of someone's compromised immune system.  For hours outside in fresh air and green spaces increase your natural killer cells which is a better protection for any virus then an immunization.  An immunization commits a portion of your immune semi permanently to only responding to that virus removing flexiblity from your immune system to respond to other insults.  For example if you have 100 white blood cells and 80% are committed to only recognizing a mumps virus in a blue hat, then when the mumps virus with the green hat arrives you only have 20% available to defend you.  Seems like an overcommitment of your resources.

Hum you can see I have returned to Emerald City feeling exuberant to get back to helping all of you optimize your health.   Once again I want to reiterate that immunization decision are individual and should be addressed with your concerns with your doctor at Emerald City Clinic.

The level of the hormone, TSH that is often used to evaluate thyroid function, varies with the time of day the samples are collected.  Collecting afternoon samples can lead to under diagnosis of hypothyroidism.

Doctors test thyroid stimulating hormone, also known as TSH, to screen for and diagnose hypothyroidism, a condition of inadequate thyroid hormone.  TSH is made in the brain and as its name implies, stimulates the thyroid gland to produce more thyroid hormone.  Thus if TSH levels increase, it the brain is telling the body to make more thyroid hormone. When TSH levels stay high it suggests that the body cannot make enough thyroid hormone.  Most laboratories define  ‘normal’ TSH as between 0.5 and 5.0 uM/L.  TSH levels above 5.0 are considered hypothyroidism and those below 0.5, hyperthyroidism.

A new study published in the August 2012 issue of Endocrine Research reveals that our current approach to diagnosing hypothyroidism has been too simplistic and needs change.  Production of TSH follows a circadian rhythm that is it follows a 24-hour cycle with regular fluctuations.  TSH production peaks at its highest level between 2:00 and 4:00 am each morning and then drops reaching its low point between 4 and 8 pm in the evening.  Though this phenomenon is well known the actual extent of these fluctuations was much greater than was previously thought.

In this current study, researchers from the Federal Endocrinological Research Centre in Moscow, tested 20 women with subclinical hypothyroidism.  Morning and afternoon TSH levels were measured and compared.    The median morning TSH value for women with subclinical hypothyroidism was 5.83 mU/L.  Their afternoon median TSH dropped down to 3.79 mU/L.  Following the current guidelines for diagnosis hypothyroidism would have been diagnosed in only half of these cases when tested in the afternoon. 

These new data explain why many people who appear to have symptoms of hypothyroidism have been told their test results were normal, that they are fine, when in truth they hypothyroid.  TSH levels measured in blood samples drawn in the afternoon should be viewed with caution, as they may be falsely low and appear normal.  Samples for all thyroid tests should be collected first thing in the morning. 

It gets more complicated than this.  Many practitioners consider the current normal range for TSH values to be too broad.  Rather than judging TSH levels above 5 as hypothryoidism, some consider the upper limit of normal to be as low as 3.0, while researchers looking at what is called sub-clinical hypothyroidism define the condition by a TSH greater than 2.5.  Apparently that is the level at which risk for cardiovascular disease increases significantly.

Combining these two bits of news, the circadian variability of TSH and the benefits of treating even slight increases in TSH, leads me to think that we should be relooking at a fair number of patients who have previously been told that their thyroids were fine.  They might not be…..

We go to the spa to feel pampered and beautiful.  Making your own spa products is easy and rewarding.  Bring the spa into your home -- come learn how to make your very own herbally inspired spa products!  During this two hour workshop you will learn how to create herbal facial scrubs, toners, masques, and steam blends; as well as body scrubs, and foot soaks.  We will discuss herbal blends for any skin type, and explore the healing and rejuvenating qualities of the various products discussed.  Handouts will include instructions on how to make a range of products, as well as a discussion on the therapeutic properties of the herbs chosen.  In this workshop, each class participant will be able to make 3 products to take home with them!!!
